Abstract
Gastric yolk sac tumor is an extremely rare tumor, with only a few cases reported in the English literature. We present a case of 50-year-old man with yolk sac tumor of stomach with adenocarcinoma without metastasis in all regional lymph nodes after he underwent subtotal gastrectomy. The tumor was early gastric carcinoma confined to submucosa. Eight months after the operation, the patient was alive without complication. Immunohistochemically, the yolk sac tumor expressed ¥á-fetoprotein (AFP), ¥á1-antitrypsin (¥á1-AT) and cytokeratin (CK), but was negative for carcinoembryonal antigen (CEA), placental alkaline phosphatase (PLAP), human chorionic gonadotrophin (hCG) and p53. The adenocarcinoma was positive for CEA and CK, but was negative for AFP, ¥á1-AT, PLAP, hCG and p53. The transitional area of yolk sac tumor and adenocarcinoma was positive for CK and weakly AFP. This findings suggest that the yolk sac tumor and adenocarcinoma components are closed related and may represent distinct phenotypes arising from a common mucosal epithelial cell.
